The DUETTO trial (ANRS) is announced... Well... Why not: Relief AND Intermittence combined. CoronaFolie obliges, it drags on... To the point of arriving after the battle, a bit like Recovery. Children, people in institutions, seniors in nursing homes, hospitalized people, etc. do not have control over their intake. They are administered to them. Synergies and reinforcement for 1/7 (or even better)


For initiation and continuous treatment, it is historically considered that 3 molecules are sufficient. Practitioners (e.g. Germans) were already doing induction/maintenance. Today too, there seems to be nothing to prevent the use of DTG x 2 /TDF/3TC followed fairly quickly by DTG/TDF/3TC then DTG (1/2) /TDF/3TC or DTG /3TC or even DTG /3TC (in 4/7) as we will soon see. Putting an extra pill, when the circumstance allows or requires it, is admissible...

Videx in addition, a futile intuition/orthodoxy?


The very first successful patient in 1/7 appears in FASEB1, and is described as being on Quad Therapy (NVP/DDI/FTC/TDF). I am not sure if the patient was on this quad before starting ICCARRE or if the boosting occurred along the way. It seems that it was like that, by chance, and that Leibo continued like that considering that the prejuduce (1 Videx more) was not very big with regard to an eventual securing.

He was keen on it, but readily admitted that there was no evidence of the futility (or not) of this reinforcement. To do the right thing (and to please the Stakanovists of the methodology), one would have had to do a good large group randomly allocated with and without reinforcement. Under these circumstances, the futility test is... futile. So not done.

For a long time, I promoted the Leibowitchian orthodoxy of the fourth molecule, aware that there was no formal proof. I have to admit that this did not convince many people. This blog is not a medical advice: you will be inspired by it, it is your choice, you will not be inspired by it, it is also your choice. And if it doesn't work, don't bother to come and bitch about it: this blog is not a medical advice, it's a double-sided cave canem: I don't encourage anyone and, conversely, no one bothers me. Right?

The fourth molecule can be useful: here is why...


For a long time, I thought that we had not done the manipulation that consists in removing the fourth molecule and see what happens. Strictly speaking, as far as I know, this manipulation has not been done. But another one... Which informs us in the same way... Except that we didn't see that it could be interpreted that way.

If the Fourth Molecule had not been removed, it could have been replaced by a placebo. That's what protocolists are like. Leibo was a doctor first and foremost, so the placebo was not his thing... The idea of replacing the Videx with plaster ... I let you judge... Well... However, Videx no longer exists, so it should have been replaced by something, a placebo? no... But...

Replacement of Videx by DTG


As Videx disappears, we start to have a good idea of the 'power' of DTG, hence the simple idea of replacing Videx by DTG. But there, there is a wolf! The literature states that NVP reduces the amount of DTG in plasma. You will be surprised to know that this is a work of Raffi(yes, the same one who... ). If we stick to Raffi's conclusion, it's not a big deal, we pretend nothing happened. Except that Raffi, he looks at 7/7. DTG fills the bathtub, NVP empties it, but since we fill it right after, the bathtub doesn't really get empty. In 1/7, we don't fill the tub a second time! And so... DTG disappears. DTG has an antidote: it is NVP!

We make NVP/DDI/TDF/3TC then we substitute, we think we make NVP/DTG/TDF/3TC, and, in fact we make NVP/DTG/TDF/3TC, i.e., a classical IRT NVP/TDF/3TC... And this? It jumps! Does it always work? No! There are exceptions, but well... We can see that with NVP/TDF/3TC, there is a benefit to supplement with DDI (or something else?)

DTG in 1/7: In TRI or in QUADRI? That is the question


All indications are that DTG counts for two: it is potent (over 4.5 Log) and it has a pharmacodynamic remanence of at least 3-4 d. Of course, we have evidence of success in 1/7...

One way to do suspenders and belt, is to take Morning and Evening. Note that in the free zone, you can find a DTG/TAF/F-3TC combo, at a very low price. In the mafia zone, not yet... Well, it's a bit of a pity... Now, the goal of the maneuver is to catch the payers (social security, etc...) on the Biktarvy ® combo. If the payer gets cheated, that's his business. From our perspective, we do not identify any difference between DTG and BTC(and for good reason!).

In practice, we read testimonials of 2/7 with Biktarvy. Why not? Now, I also don't see what we have to lose by reinforcing a bit... With what? RPV, ATV (without booster, since the usefulness of booster in 1/7 is far from being obvious: futile or even counterproductive, in this context), etc.

Islatravir: addition or substitution?


Especially sinceIslatravir is coming. Well... Overdosing seems to be avoided, on paper(too bad... ). Well... The dilemma of the 4th molecule is there in front of us: Islatravir® is not very powerful (1,5 Log) but very persistent (10 days at least!). It makes you want to use it. As an addition or as a substitution? For me, it will probably be used to replace one of the molecules that I probably have too much of. For those who stayed in IRT all the way down, the question will be interesting. Might as well start thinking about it!

- A controversy (pro and anti HCQ), a randomized trial and each side claims victory! This is normal: in order to be published, the authors conclude that it doesn't work... This allows them to reveal their data, which show... that it works!

It's interesting to hear such good news from people who have read a lot of us... There are 3 types of PTCs (post treatment controlers): those that we find by chance (rare), those that we find by looking where we think we find them (Viscontis, Antwerp ...) and those of intention, who have undertaken a process in which control is possible. This is what interests us most!

The commentary is timely, since here is one who controls, but is afraid and resumes treatment ... This is the topic of the day...


A controller identified by chance...


Here, we will dwell and, for lack of a better word, embroider a bit, on what may have happened in a study in our Eclipse bibliography. Sneller (2020), takes chronic patients under effective treatment and studies the dynamics of rebound. There, Surprise!
One participant (#03), who spontaneously controlled his plasma viremia, resumed ART even before meeting the criteria for restart.

This patient is indeed found in the table, restarting treatment at week 13.

A PTC that falsifies this study, and the others...


We had seen that in the Davey Study, a very long Eclipse patient was distorting all the statistics. It lengthened the mean and especially the standard deviation(one should be wary of automatic calculations...). We had the table, we could redo the calculation. Here, it is the other way around: the patient takes the initiative to resume the treatment, thus shortening the recorded duration, integrated in the calculation.

The control arm tests are distorted


As we saw in the Levothyrox case, comparing population A to population B only makes sense if the standard deviation AND the range are both narrowed. In the 'modern' patient, this is no longer the case: we have a little of everything: between 1 and > 4 weeks

The Thirst for Glory incites to deceive the patients


Control arm trials are flawed, and sometimes unethical. Not the trial itself, but its preparation: an example is given by a Spanish study: Extremely low viral reservoir in chronically HIV-1 infected treated individuals.
What is the point of having a weak reservoir if not to be able to try PTC or, even better, Eclipsotherapy? Having a long Eclipse and treating 7/7: how stupid!

It is not difficult to imagine that the constitution of registers of patients pre-selected for trials is not compatible with explaining them on 4/7 (or even better). In order to do medical science, doctors are probably tempted to keep silent about reasonable alternatives...

On the road to recovery, there are...


Either the PTC or the Eclipsotherapy... Both allow to simulating a cure. Including seroreversion. Imagine: No RNA in the blood, no DNA in the reservoir, no residual meds in the body, and a negative serology. A sweet unattainable dream? Not sure, we'll see that another time...

Some anti-COVID news

This should piss off more than a few people, and I'm surprised that pharisaical vetrology hasn't managed to get it cancelled yet... To be continued...

Quatuor essay:
- EudraCT(2017-000040-17) indicates that the trial has been completed.
- ANRS informs that the lead article is in preparation.(press release of 11/30/20)
As a reminder, the objective is to evaluate the non-inferiority at 48 weeks of the strategy of taking antiretroviral treatment on 4 consecutive days out of 7 versus the strategy of continuous treatment 7 days out of 7, in patients with virological success on antiretroviral treatment (CV<50 copies/mL).

The results of the comparative phase, the only one allowing to rule on non-inferiority, are published. The authors concluded that the trial was non-inferior. The trial was continued to include all patients. The 2 arms being in the intervention, this continuation does not contribute to the comparison.

- New entry in my Eclipse bibliography: a CASTAGNA study, from 2019, with 9 patients, and a mini Eclipse of 2 weeks, mean at 21 d. As usual!

My new rhythm: the 1st and 15th of the month


My old rhythm was every other Monday, so 1/14. So 26 weeks.

It's an interesting concept: the day of liberation is a known concept, it's the day from which we stop working to pay taxes and we start, finally, to work for ourselves. There was a time when my day of economic liberation was very late in the year: I spent my life feeding the mamouth!

Reduce the number of alarms


In 1/14 mode, I had 3 alarms: the 1st of the month, to check that my administrative tasks are done (accounting, declaration, invoices, etc...) and 2 alarms programmed to follow my medication intake and be sure not to forget anything. Around Sept 2020, I merged everything: I set an intake on the 1st of each month (and its general alarm) and I have a specific alarm on the 15th of the month. The alarm on the 1st of the month is general, the boring things to do, so one more or less boring thing... There is no stress... So I only have one alarm on the 15th.

I play the little train


For some time now, I've been taking meds every day (we manage the after-effects...). They are incompatible with ARVs! Hence the medical interest of the 1/15. I don't take them the day of the ARVs and that's all, it's simple and efficient.

Healing, even at a mini level, is too serious a matter to leave to forgetful doctors. The pillbox is your friend... Mine is not ordinary... I use the pillbox with removable case from MUJI. I have several tens of them, that is to say about fifty hundred boxes. I prepare my boxes well in advance and I assemble the little trains once a quarter. No, no, I don't fill them every week... Every week I take out the pillbox, neatly arranged in s��quence and basta: I only handle the medocs once every 3-6 months.

No stress, no pathos, no forgetting. Especially since with Dodeca, you have to do things right (there is an order to follow).

The days of liberation according to the rhythm


Where we understand that it is more attractive to go from 4/7 to 2/7 than from 1/15 to 1/21!... Does the healing escape us? Yes, maybe... And then?!

What has become of my tank, that so close I had held?


It's gone!!! So there... Well, I don't think much of residual DNA measurement, Rouzioux style... But hey... I do it every year, to see... If it went up a lot, it would be interesting. The value itself does not help us much, but maybe the trend... In a pinch... Well... We don't know... So we measure with the tools we have...

Well... The surprise of the surprise, it is that we can't do it anymore...

No reservoir (in the sense of Rouzioux) does it mean no virus? no resurgence in case of stop? Not really... The very few who succeeded in PTC (post treatment control) had a non-measurable reservoir, but patients with an ultra-low reservoir (non-measurable in standard) nevertheless saw the virus resurface. Simply, we have seen our detractors raise the spectre of an enrichment of the reservoir, but I do not see this. This allows me to put this argument of the "anti-cure"(all the Gileadolaters, all the Pharisee verology, etc.) in its right place: the trash can...

What is amazing is that the disappearance of the tank is 6 months after my unappealing failure at 1/7 in DTG 150 mg... We already know that: doing an ATI (analytical interruption), for example, has no measurable consequence, at 6 months, on the DNA reservoir. In this case, it was not an ITA, but I was not very worried either. With good reason: now it is verified.

No tank ... Not a huge ratio either


The CD4/CD8 ratio moves very slowly, at the rate of the eventual disappearance of supernumerary CD8. Knowing that their half life is 9 months, it's not worth monitoring every 5 minutes... But anyway... It is stable, very slightly above 1. A little too slightly for my taste to resume our long march towards 1/30 without worry. I'll wait a bit, especially since I don't have access to the CVs anymore, so it's quiet...

Do my anti-tank actions have anything to do with it?


What anti-tank actions? It's simple, I make a list of all the proposals that have been made here and there, I keep only those that are accessible to me and I go... Which one do I choose? None, I do them all... Hoping that one of them will work. It's the cocktail strategy, and even mega-cocktail.

There are in-vitro manips that eliminate the r��servoir. Then comes the question of in-me practice. There, for the record, I may have inadvertently r��ceeded in implementing the Weinberg proposal. We'll see about that some other time....

The Eclipse has lengthened: from a few days to a few weeks...

At the very beginning, when I became aware of the existence of an alternative to overmedication, I kept reading and rereading the same pages: Faseb, Valas, Pictorus... The emergence was that the information was rare... With Internet, one could find... It was necessary to get a little wet... So the more entry points you have, the better. For the curious mind, ICCARRE can be a door to Eclipsotherapy, which is the inevitable extension of it.

The Eclipse : subject of research...


If, one day, a research group is launched, the first work will be to establish the state of the art of knowledge... In a word, to make a bibliography. It's long and tedious, nobody really wants to do it, especially when you think you've done everything. But it is not the case! It's time consuming and needs to be maintained. Well... Once it is done, we understand a little better.

Convincing visualization of the biblio


So I took my biblio, and classified in a table, by year of publication, indicating the number of patients and the characteristics of the observed Eclipse. Then, I interpret the mini to estimate if such or such strategy is reasonably possible or not. The capture link allows quick access to the biblio verification by exposing the part of the reference that documented the table. This is the most difficult part: the data we are interested in are often buried in another discussion, typically the control patients in a vaccine or shock-and-kill intervention. I have retained only the control patients to avoid any objections. Since the interventions studied had no effect whatsoever, they could have been legitimately included. But then again... Given the large number of cases listed, this was not necessary...

The cliff and the continental shelf


Wherever you start from, if you walk towards the west, you will reach the sea. If you arrive by the cliff the fall will be brutal; by the beach, it is safer. The Eclipse, at its beginning, was short, for almost everybody: the bell curve was very tight. Except for 2-3 days of clear weather, we were soon facing a wall. This wall was a collective one, since, more or less, everyone was entitled to it, in similar times. It was enough to play the penguins at the edge of the ice floe. Nobody dared to do it, but if the first one survived, we threw ourselves into the water. and again, until the hecatomb started. So there was a particular interest in looking at what others were doing... The ICCARRE study is that... The precursors move forward and where the danger becomes a reality, we stay away.

By multiple attempts, a strategy is revealed that is better than the others. You should observe the first flock of sheep. Then, one can protocolize on a choice of molecules and a predefined rhythm. It's 4/7, and OMNIBVS. This is true if we are all subjected to a similar risk, let's say identical to make it short. But today, this is not true!
If the wall is at D+4, for all or a large number, we all go until D+2 or D+3 and basta, we can hardly hope for better. Dr Cal COHEN (FOTO) validates the 5/7 and invalidates the 4/7, because of failures under PI (Kaletra®...) and gives up the game. The gain is modest and it is denigrated as negligible (which will have been prejudicial). Leibo goes further... He sees that Kaletra® and Isentress® do not work, but perseveres with NNRTIs, in particular NVP, which has the longest persistence. He attributes the feasibility of the 1/7 to the sole merit of his magic NVP/DDI/TDF/F-3TC combo.

ICCARRE is on the way to obsolescence


In 2020, the observed lengthening of the Eclipse makes ICCARRE in the process of obsolescence: the mean has shifted and the standard deviation strongly lengthened: eclipses of 4, 15, 21, 30 days are not rare anymore. In fact, one can speculate that it is not the eclipse that has changed but the population. By treating better, earlier and stronger, the population is enriched in favorable cases. Have the disadvantaged of history disappeared? Who knows... They are a minority and will perhaps 'jump' to 3/7, 2/7... Certainly... And then??? For the patients, more and more numerous, with long eclipse, the past experience of the patients with short eclipse does not teach them anything. Even worse, the protocolist approach deprives them of the formulas of quasi-healing.

On a beach, the risk of sinking, suddenly, at the next step is very low, as long as one progresses cautiously, well aware that one is not going to cross the ocean any more. The marginal risk of going from X/7 to (X-1)/7 is lower than the non-inferiority limit. So 3/7 is not inferior to 4/7, 2/7 not inferior to 2/7, 1/9 not inferior to 1/7, etc. Because the slope is gentle...

We can also reasonably think thatISLATRAVIR will change all that.

The very idea that a selection bias may exist is, in fact, an argument in favor of Eclipsotherapy. We'll see about that next time...
